Masca Medical Aid Society currently has schemes for companies and groups of individuals that are five or more.
To join please download all three application forms that are below. Print the forms and fill in your details. You can submit your form at any of our branches.

Please Note

Application form 1 is to be filled in only by one individual that is the company's representative or the group representative.
Application form 2 is to be filled in by all applicants.
Application form 3 is to be filled in by all applicants.
